First of all, the guard has to make sure the prisoner isn’t “dieing”. They need to run more tests, Or just question him. A dead prisoner is pretty much useless. So yeah, if a guy is coughing, and making puking sounds, of course you’d check, though in the guard’s fault, he should of been armed with a gun or some sort to attempt to defend himself.
Also, in a world of Hackers, anything is possible, so you can never be fully trained except for quick wit and reflexes. If their soldiers don’t have that, then yeah, the boss is the one that should be called stupid.

I’d blame the boss guy in this situation.
